Service Description

The Monthly MindCare & Wellness Membership provides comprehensive mental health support to members and their caregivers. After an initial face-to-face examination and wellness consultation, members receive a range of benefits, including:

Monthly Wellness Touchpoints: These visits offer additional support and resources beyond regular appointments. Touchpoints can be conducted via video chat, phone, or in-person and focus on lifestyle, stress management, and self-care. Members can choose between monthly 60-minute visits or bimonthly 30-minute visits, and caregivers and family members are welcome to participate.

Unlimited HIPAA-compliant text support: Members have access to secure, confidential text support between 9 AM and 9 PM daily. While responses may not be immediate, members can rest assured that their provider will respond as soon as possible.

Same or next day appointments: Members and their caregivers can schedule appointments with their provider on the same or next day, based on availability. Please provide at least 24 hours notice for cancellations.

Direct line to the provider: Members and their caregivers have up to 60 minutes of urgent or non-urgent calls per month with their provider.

Access to the provider through electronic communications: Members can reach out to their provider through electronic communications for support and advice.

Extended hours: Members can schedule two after-hours (6 PM - 9 PM) or weekend visits or calls per month for added flexibility.

Trip fee waived for house call visits 1 & 2. Trips 3+ discounted 50%.

Care coordination: Members receive comprehensive and seamless care through collaboration with other healthcare providers.

Caregiver education and resources: Caregivers receive education and resources on stress management, communication skills, and navigating the healthcare system.

Support group or community: Members and their caregivers have access to a support group or community to connect with others in similar situations and receive additional support and resources.

Referral assistance: GNC will assist members with referrals and any necessary tests.

The Monthly Mindcare & Caregiver Support Membership provides peace of mind and comprehensive mental health support for members and their caregivers.
